1. Fill-out the Membership Application and Data Privacy Consent Forms;

2. Pay the Annual Dues of Php4,500.00 (if already paid, please proceed to STEP 3)

c. PRC Certificate of Registration (CoR) (scanned document should be in: PDF, JPG, or PNG format)
d. PRC ID (front side) (scanned document should be in: PDF, JPG, or PNG format)
e. PRC ID (back side) (scanned document should be in: PDF, JPG, or PNG format)

MEMBERSHIP PROCEDURE:
|
| |
1. The application will be verified and evaluated by the Membership Committee;
|
| |
2. The application will be circulated to the membership for possible objection and comments;
|
| |
3. Interview by the Board will be conducted for further inquiries;
|
| |
4. After 30 days from the circulation of the application,
the same will be deliberated in the Board for the approval of the said application;
|
| |
5. Briefing, and;
|
| |
6. The approval of the application will be circulated to the membership.
