Member Data Privacy Consent Form

By clicking on the "I AGREE" checkbox, I confirm that I am giving consent for REBAP Global City Chapter to collect and process personal data in relation to my membership in this organization;

I also warrant that I have read and understood the following provisions and agree with their full implementation:

1. Republic Act No. 10173, or the Data Privacy Act (DPA) of 2012, and other relevant Philippine laws apply to the collection and processing of my personal data. "Personal data" refers to all types of personal information "whether recorded in a material form or not from which the identity of an individual is apparent or can be reasonably and directly ascertained by the entity holding the information, or when put together with other information would directly and certainly identify an individual." (NPC Advisory No. 207-01);

2. I understand that in joining the organization, I am allowing REBAP Global City Chapter to collect, use, process and share my personal data where a legitimate professional and organizational interest exists in its determination, including professional, historical and statistical purposes. Personal data include my personal circumstances and directory information such as - but not limited to - demographic profile, contact details like addresses, landline/mobile numbers, email address, professional records. Information will also be collected on my professional records, attendance in all organization’s meetings; information for and in relation to REBAP Global City Chapter activities such as seminars or webinars, trainings, participation in all levels of the organization and international activities. The personal data may be in written form, pictures or videos, recordings from all types of cameras for security purposes or other forms;

3. My personal data will be accessed and used by and its Officers concerned who have a legitimate interest in it for the purpose of carrying out their organizational and professional duties;

4. The purposes for which REBAP Global City Chapter may share or disclose my personal data include among others:

(a) Publishing organizational activities, projects, services, accomplishments in the REBAP Global City Chapter office, website, social media sites and publications;

(b) Promoting the organization in all sorts through photos, videos, brochures, website posting, newspaper advertisements, physical and electronic bulletin, and other media;

(d) providing public and private institutions, government agencies, public or private corporations or the like, upon their request, for purposes of professional dealings for Academic Purposes only, and;

(e) sharing of photos with the press upon the latter's request for the purpose of posting these on social media;

5. Unless otherwise provided by law or by REBAP Global City Chapter Board of Trustees’ approval, my personal data will be indefinitely retained for historical and statistical purposes. Any physical documents collected shall be stored in a secured-place, and only authorized personnel is/are given the authority to access the data. Where a retention period is provided by REBAP Global City Chapter, all records after such period will be securely disposed of (digital and physical).

6. Under the DPA 2012, the data subject has the right, among others, to be informed of the processing of her/his data, reasonable access to the personal information upon demand, correct, withdraw or order the blocking, removal or destruction of her or his personal information on grounds indicated therein;

7. By providing personal data to REBAP Global City Chapter, I am confirming that the data are true and correct. I understand that REBAP Global City Chapter reserves the right to revise any decision made on the basis of the information I provide should the information be found to be untrue and incorrect, and;

8. Any issue that may arise in connection with the processing of the personal information will be settled amicably with the organization before resorting to the appropriate arbitration or court proceedings within Philippine jurisdiction.
